Appointments and Referrals
If Occupational or Physical therapy services are needed, a physician referral for treatment is required.

Arrive 15 minutes prior to your evaluation session to complete your registration paperwork. Items to bring with you to your appointments are: post-surgical discharge information, your insurance card, list of medications, wear loose fitting clothes/or bring shorts. Shorts and gowns are available if needed.
On average, your first appointment (evaluation) will last approximately one hour. The length of time and frequency of subsequent treatment session require will be determined by the therapist (ranging from 30-60 minutes).

Cancellations - We understand life happens and sometimes an appointment cannot be kept. Please notify Cornerstone as soon as possible if you are unable to keep your scheduled appointment
Insurance
Cornerstone accepts most major insurances as an In-network provider. We recommend you contact your insurance company to check your coverage.

Our office manager will verify your insurance benefits and collect co-pays and/or other fees per your coverage. We accept credit/debit cards, check or cash at the time of service.
