Do I need a Doctor’s referral?

All Medica, Medicaid and Medicare patients are required to have a doctor's referral. 
Occupational therapy requires a physician's order regardless of type of insurance. 
Some insurances need prior authorization to receive service.

What should I wear?

All patients are asked to wear loose, comfortable clothes. Patients that are coming for physical therapy may also need to wear shorts. If you are unable to bring either of these items, we do have shorts and gowns to let you borrow during your appointment. If you recently had surgery and were put in any kind of bracing, we ask that you continue to wear the brace to therapy as long as your physician advises.

How long is each visit?

Initial evaluations may last up to an hour. Most patients are scheduled for 45-minute increments after the initial visit. For the first visit we request you to come approximately 15-20 early to fill out any necessary paperwork.

What do I need to bring with me?

For the initial visit, you should bring your post-surgical discharge information, insurance card(s) and a form of payment to cover your co-pay/deductible (if applicable). You also need to bring along your listing of medications.
